Am I right that the only room to remain exactly the same appearance/theme wise and name-wise though the whole run of the show was Shrine of the Silver Monkey? The Ledges were always there but the name alternated between Ledges and Cave of Sighs.

Yes, the Shrine is the only room in every episode. The Ledges are different enough from the Cave, because they sealed off the front entrance and made people go through the gate first.
-
One could argue that the Shrine's appearance changed a lot between seasons, but besides that minor truth, nothing changed. Same name, same objective, same doors, same statue-assembling-challenged kids... It has really come to be a symbol of the show.

The appearance of the whole temple changed a lot, though. But you're right, there are little differences (like the monkey noise after they finish the statue in Season 3).
Olmec's pronunciation of the room name went though some big changes as well.

The room also became darker and the smog effect was added to it as the seasons progressed. They also got rid of The Treasure of Anne Bonny, which was sitting on one of the "cabinets".
